Accumlation and Aggregation

General Introduction

Accumulation and aggregation are a disease and syndrome of mass in the abdomen, with distention or pain. Accumulation and aggregation have some differences in sevious extents of disease and pathologic changes. Accumulation has a form and fixed parts of distention and pain .It belongs to a disease of blood system ,and it is a disease of Zang-organs; aggregation does not have a form, without fixed parts of distention and pain. It belongs to a disease of Qi system and it is a disease of fu-organs.

Generally, aggregation disease is milder, with shorter duration of disease, so it is easily to be cured. Accumulation disease is more severe, with longer duration of disease, accumulating as mass, so it is difficult to be cured.

In   western,medicine gastrointestinal dysfunction, imcomplete intestinal obstruction, pyloric odstruction, hepatosplenomegaly, visceroptosis, intestinal tuberculosis, abdominal swelling and pain, etc all can be treated besed on differntiation of symptoms and signs referecing this syndrome.

Causes of disease and pathogenesis

Occurrence of  accumulation and aggregation have a process of gradual production and development. emotional disturbance, impairment, of improper diet, attack of excess Qi and weakness after illness, or lack of treatment and delayed treatment of jaundice, hypochondial pain, malaria lead to protracted illness and injury of the liver and spleen, inccoordination between viscera, stagnation of Qi, blood stasis in the interior, or with stagnation of phlegm and dampness forming accumulation and aggregation. Occurence of accumulation and aggregation is re lated mainly with the liver and spleen. Generally speaking, aggrgation syndrome is caused mainly by stagnation of Qi, and aggregation syndrome by retention of blood stasis, but both may be accompanied by phlegm and dampness. Because Qi and blood are correlated each other, protracted stagnation of Qi can induseblood  stasis,and failure of removing blood stasis also can cause stagnation of Qi,so,accumulation and aggregation have differences and correlation in pathogenesis. protracted aggregation syndrome induces often accmusation syndrome; unability to remove accumlation syndrome often occurs distecding pain.In addition, protraceed accumulation and aggregation can induce dysfuntion of the liver tion,protracted accumulation and aggregation can induce dysfunction of the liver and spleen,leading to deficiency of vital Qi, therefore ,this syndrome is often excess syndrome at initial stage ,and often deficiency syndrome at chronic stage ,or deficiency with excess. Casues of disease and pathogenesis of accmulatoin and aggregation syndromes are discussed in the following .

I£®Emotional disturbance, stagnation of Qi and blood stasis; melancholy and over thinking impair the liver and spleen Injury of the live induces stagnation of Qi. Qi is the commander of blood, protracted stagnation of Qi can incduce blood stasis. After prolonged stagnation of Qi and blood stasis can form accumulation and aggregation ;injury of the spleen causes dysfunction in transporting and transforming nutrients, failure of food producing nutrients forms phlegm pathogen, obstruction of blood vessels by phlegm pathogen results in unfree flow of blood, stasis of phlegm and blood leads to accumulation and aggregation in the abdomen.

II£®Improper diet, production of phlegm pathogen, drinking or eating over fat, sweet, fried, pungent and hot food;or improper diet ,over hungry or full, irregular daily life induce injury of the spleen and stomach, dysfuction in transporting and transforming nutriets, leading to production of danpness pathogen in body and coagulation of phlegm, and phlegm causing stagnation of Qi. Incoordination between Qi and blood, and interaction of Qi , blood and phlegm form accumulation and aggregation a in the abdomen.

III£®Attack of pathogenic factors, and retention of pathogenic factors; Cold pathogen or damp-heat pathogen and other exopathogens invade the body , retenting in the abdomen and forming aggregation after long time, and then causing accumulation.

Iv. Jaundice, hypochondial pain, malaria, infection by blood fluke, etc; Stagnation of dampness pathogen, incoordination between Qi and blood induce incoordination between the liver and spleen, and interaction of stagnation of Qi and blood stasis and phlegm pathogens forms accumulation and aggregation in the abdomen.

To sum up, this disease and syndrome have many causes of disease, but the part of disease is mainly in the liver and spleen, and its  pathlogical changes are closely related with stagnation of Qi, blood stasis and coagulation of of phlegm. Formation of the syndrome also is closely related to strength and weakness of vital Qi .Generally, at initial often caused by sagnation kf Qi, retention of dampness and blood stasis, at chronic stage, it is often caused by interaction of phlegm retention and blood stasis, and deficiency of wital Qi. dysfunction of the liver and spleen. or burning injuy of blood vessels by heat retention may induce bleeding; retention of damp-heat in the middle-Jiao invades the liver and gallbladder, jaundice may occur retention of water and dampness may induce fullness and sweeling in the abdomen, and other syndromes.
